Welcome! On your first visit to Ankle & Foot Associates you will be asked to fill out patient forms in order to provide us with your personal information and your medical history. In order to save you time, please print the patient forms from our web site, complete and bring them to your appointment. You will need to bring your insurance cards to your first appointment so that we may take a copy for our records.

What to Expect
Most patients have some form of health insurance that may pay a percentage of the cost of the care provided. When you provide us with current and accurate insurance information, we can successfully file most insurance claims on your behalf. Please check with our office when making an appointment to verify whether Ankle & Foot Associates accepts your insurance carrier.

Patient's covered under Workers Compensation insurance should present all information and records associated with a work related injury and claim one week prior to their appointment. This information includes:

  • Policy Number
  • Date of Injury

If you are enrolled in a managed care insurance (HMO), it is your responsibility to obtain a written referral or prior-authorization from your Primary Care Physician (PCP) before your visit with the doctor.

Some of the cost of your care may not be covered by insurance, such as co-pays, deductibles, coinsurance and "non-covered services". We require that you remit payment for any co-payments and non-covered services at the time of your appointment. We accept cash, Visa & MasterCard. Policy deductibles, coinsurance and non-covered services will become the responsibility of the patient once we are notified by your insurance carrier.

Patients with no health insurance will be expected to remit your initial visit costs at the time of your first visit. Our Office Manager can meet with you , if needed, at your first visit to calculate your total costs, establish an acceptable payment plan and sign and Extended Payment Plan.
