If you are enrolled in a managed care insurance (HMO), it is your responsibility
to obtain a written referral or prior-authorization from your Primary Care
Physician (PCP) before your visit with the doctor.
Some of the cost of your care may not be covered by insurance, such as co-pays,
deductibles, coinsurance and "non-covered services". We require
that you remit payment for any co-payments and non-covered services at the
time of your appointment. We accept cash, Visa & MasterCard. Policy deductibles,
coinsurance and non-covered services will become the responsibility of the
patient once we are notified by your insurance carrier.
Patients with no health insurance will be expected to remit your initial
visit costs at the time of your first visit. Our Office Manager can meet with
you , if needed, at your first visit to calculate your total costs, establish
an acceptable payment plan and sign and Extended Payment Plan.
